EXCELLENT article. We are looking for an HD solution for our mid-sized church, and this article really helped and confirmed some of what we'd already found – plus, it opened our eyes to some other options. Thank you.

Using two Blackmagic HDMI cards as well as two basic HDMI switches feeding the cards ($99.00 each)you could do a 4 camera config with the On Air soft switch. Kind of a kludge to manually hardware switch before you use the 2 channel soft switch, and certainly no seasoned broadcast person would put up with this, but for a low cost starter like me a 4+ HD camera switch under $700.00 is very appealing

You don't have the Focus HX-1 or HX-2 along with their wonderful MCSDI-1 listed. Nor do you mention the easy and affordable solution of adding FW ports (either tower or laptop) and using VeeScope Live or HD Monitor Pro as a valid solution. There are many more options out there besides these, and some of these, IMHO, are not worth even mentioning due to the limitations you point out.
